Subjective Exam
This concerns the medical history — the reconstruction of the case from a clinical perspective. For a successful consultation, it is essential that the patient reports everything related to their ocular health history and any visual symptoms. An effective subjective exam also requires the active cooperation of the patient during the visit.
Objective Exam
This is the phase involving tests and examinations using the most appropriate tools. It consists of collecting the data necessary for an accurate optometric evaluation.
Problem Identification
This stage involves analyzing the subjective and objective information to determine the most suitable therapy. It is generally divided into three sub-categories: refractive errors, binocular vision issues, and eye health screening.
Action Plan
This is the final phase, in which the optometrist chooses the treatment or program to be carried out with the patient.
This method, known as S.O.A.P. (Subjective, Objective, Assessment, Plan), is a clinical approach considered highly reliable and is followed during every visit, tailored to the patient's needs.
